Thanks for the post. However I followed instructions and it kept loading to a white blank screen…when i hit refresh it keeps taking me back to the US form so I just filled out the french version. Hope it works :p

There was no CANADIAN — ENGLISH on the site, I had to click on French and go through the French site to find the promotions page and then do it, but I was finally able to get my form filled out… here’s hoping it works

I found the Canadian tab very easily. It was right between US samples and the tab in French.
