   their is no reason for a company to require a phone number unless they are selling it, I’ll take a pass on the samples of this one.

   The form doesn’t have a ‘submit’ button. I’ve loaded the page several times but still no ‘submit’ button. I’ve even emailed them but have gotten on response.

   @Lindsay, there is a submit button when i did it and i just checked and there is one there now.

    They most likely do not offer this to current members because they’re using it as a way to get new members.

    Initially I went to the site using Firefox and the submit button wasn’t there. Then I tried IE and at first the button wasn’t there, either, but then I highlighted the “Please Note…” section at the bottom of the form with my mouse and kept dragging the mouse down when all of a sudden the submit button appeared! Try it and see if it works for you..

    I tried it and there was no submit button, i went back a couple hours later and there was one, maybe there was a temporary problem. Thanks Uechi Boy for the 555 5555 idea, i never thought of that.

    Your package should arrive. It takes a couple of months. They had this promotion about three months ago or more and I requested a package and got mine.
