I have tried to complete the questionaire several times &, although it states that if, I do not have a physician I should enter NA, I am not allowed to proceed if these blanks ane not filled in. What can I do to proceed?
This is not what it looks like I have never revived it and every time I have tried all I get is tons of junk e-mails not worth it .

i have sent for awhile back..and still have not got it..if your not going to send them then delete you add
