You do not need specially marked packaging, just the UPC and keep your receipt as the form asks for the receipt information including receipt number.
It gives you the option of receiving a print or digital subscription. If you choose digital, you will receive the current available issue. If you choose print, you will receive your first issue in 4-8 weeks. If you are currently a Canadian Living subscriber, enter your information exactly as shown on your account and the 3 month subscription will be added as an extension of your current subscription.
