This is a Canadian company with an online ordering interface–just upload your image or use the tool to make simple cards online. They have templates available if you aren’t artistically inclined.

I ordered these myself, and the cards came on the day they were promised, in perfect condition on very nice glossy paper stock. Highly recommended.
