Could not put the year in, the screen shows it only for a second, then it disappears. Tried filling out ‘male’, but it made no difference. Cannot fill out the first page to submit.

Once again, after completing required information, nothing available. I appreciate the opportunity to receive something free, however, I deplore your taking information from participants and then slamming the door shut. Not the proper way to do business.
