It didn’t work for me the first 3-4 times I tried to submit it. Then it said “time out error” and I refreshed the page and got the same message above: “Your sample will arrive in about 3-4 weeks.”

I filled the form out 3 times. It kept saying, we need a valid email adress, now I don’t know if it has my mailing adress as I filled in my email adress in that spot so it would process!
