For those who have or had peanut butter that was affected by the recent Jif recall, there is now a form that you are able to use for reimbursement from the company. To fill out the form you can click the link below.

You will need to know the UPC from the affected product, but if you no longer have the product you can simply select the product from the drop down list. The Canadian products are near the bottom of the list, so once you click be sure to start scrolling to the bottom. If you had different products, select the first UPC and the amount that you had, then select the next UPC and the amount, etc. You may choose up to five different products, and if you had more than five you can submit another form.

Once you have submitted the form you will receive an email letting you know that you will be sent product replacement coupons.
