Tax is returned on the $30 purchase only. Offer valid for product(s) purchased from 9/1/13 – 9/28/13. Your request must be postmarked by 10/28/13.   Mail the  completed form along with original dated sales receipt with store name and product purchase price circled. *All purchases must be on the same receipt* Include the original UPC(s) from the product. If UPC is not able to be removed, the UPC must be written in the spaces provided on the form.  One per person, address or household.
