I found the booklet at Fortinos, and several others have found this at other Loblaws banner stores on the coupon board. The first rebate form in this booklet is for up to $15.99 back by mail when you purchase any Finish Quantum Max 18-64 count, Finish Quantum Max Lemon Sparkle 18-64 count, or Finish Quantum Max Power & Free 18-64 count. The second rebate is for up to $10 back by mail when you purchase Finish Jet-Dry, Quantum Jet-Dry, or Finish Dishwasher Cleaner. The last valid date to purchase these products is January the 5th.

Also, these rebates are not store specific even though for the most part the seem to be found at Loblaws banner stores.
