To claim your grooming kit you must follow these steps:
- Purchase a 10.89 kg or 13.61 kg bag of NUTRO Dog Food between 4/1/15 and 7/2/15.
- Print and complete the official rebate form.
- Remove the proof-of-purchase/Universal Product Code (UPC) symbol from the outer package of your 10.89 kg or 13.61 kg bag of NUTRO Dog Food
- Circle the purchase price on your original cash register receipt that’s dated between 4/1/15 and 7/2/15.
- Mail your completed rebate form, proof-of-purchase and the original, dated register receipt to: P.O. Box 13469, Saint John, NB E2L 5E7
