Purchase one box of Quaker Harvest Crunch Quinoa Chocolate Nut Medley Granola Bars, and get a rebate of up to $3.99 by mail with this form. Purchase must be made between June 3rd and September 30th, 2013, and mailed by October 31st, 2013. There is a limit of three mail in rebates per household, but each rebate must be mailed separately and must be accompanied by the above form. The info above is incorrect. I emailed pepsico and they told me that you need separate receipts and rebate forms for each box BUT they can be mailed in one envelope~
