To participate in this offer, you must be registered with SmartSaver so you will have to first take care of that, and if you are already signed up you can simply log in. You must print the offer form and mail it with your receipt in order to qualify for the rebate. You have until October the 30th to purchase the product, and your submission must be post-marked by November the 30th. The maximum that you will receive is $9.49 to cover both the cost of the product and taxes, so keep that in mind when you are making your purchase.

There are a limited number of rebate forms available so print yours as soon as possible or you may miss out on the offer.
